Research shows that about 90% of herniated disc cases — many of which involve annular tears — go away on their own or can be remedied by conservative treatments within 6 months. However, when a tear does become painful, it’s usually because inflammatory chemicals irritate nearby nerves or because the tear is contributing to a bulging or herniated disc.

Types of Annular Tears and Their Impact on Your Spine

Annual tears are often classified by their location within the spinal disc:

  • Central tears: Occur toward the middle of the disc
  • Paracentral tears: Form slightly off-center
  • Lateral tears: Develop near the outer edge of the disc

In one study of patients undergoing surgery for annular tears, about 80% were central, 12% were paracentral, and 8% were lateral. The location matters because it can affect symptoms and outcomes. For example, paracentral tears tend to have poorer recovery outcomes, with disability scores improving less than those of other tear types.

However, annular tears may also be described by the pattern of the tear, which often helps explain why some tears hurt more than others:

  • Transverse or peripheral tears: Start at the outer edge of the disc. This is where many pain fibers live, so these tears are more likely to cause noticeable pain.
  • Radial tears: Begin deep in the center of the disc and spread outward, often causing few or no symptoms at first.
  • Concentric tears: Form in circular rings around the disc. These are commonly linked to twisting injuries (like swinging a baseball bat or a golf club, for example). 

Common Symptoms and When to Seek Treatment

When annular tears do cause symptoms, these may include:

  • Deep, aching back or neck pain
  • Pain that worsens when sitting, bending, or twisting
  • Sharp pain that radiates into the arms or legs
  • Tingling, numbness, or weakness

Studies show that patients with annular tears may experience more intense and longer-lasting nerve-related symptoms after surgery compared to patients without tears, so accurate diagnosis and treatment planning are important. 

You should seek medical evaluation if your pain lasts more than a few weeks, interferes with daily activities, or is accompanied by weakness or numbness. Early evaluation can prevent the condition from worsening or help you avoid more invasive treatments.

How Annular Tears Are Diagnosed in Beverly Hills

I had a disc replacement almost a year ago. I feel much better. I chose him to be my surgeon because he came highly recommended from several different sources. Last month, Dr. Melemed Noticed something wrong in the current x-ray compared to a few months ago. I had zero symptoms and thought I was just fine. He quickly asked to redo the surgery and explain to me what might've happened. The surgical center he sent me to both times have been amazing and I really appreciate the Dr reaching out to me even though I had no symptoms. That speaks highly of his character.

I was incapacitated, couldn’t stand, sit or walk. The pain was excruciating. I’ll spare you the diagnosis, but it was serious. I could only lie down with my foot up to reduce the pain. It required EMERGENCY surgery. I consulted a couple surgeons that didn’t give me any confidence. I got referred to Dr. Melamed. During my initial consultation, he nailed the source of my pain before I could even tell him. His confidence, manner, and his staff made the decision for me. I was scheduled for surgery within 4 weeks. The day before my surgery, I became ill with a flu bug. I was heartbroken to have to cancel, but he moved heaven and earth to get me in within a week. And that took some doing. Dr. Melamed did 2 procedures in ONE day. Total of 5 hours. The next day, although a bit premature, I had the nurse help me get to my feet. In a miracle, I could stand, I could walk, I could sit. There was some rehab time, but as I sit here today, I am 90% recovered. He is a surgeon who knows his craft. His staff was incredible always on point. He used minimally invasive technique to access the lumbar. His pain regiment did NOT include narcotics, and it worked GREAT. Dr. Melamed and his staff are at the top of their game. What more can I say?

Dr. Melamed is above and beyond the finest physician, person and surgeon I have ever encountered or could envision. On three separate, unrelated cases, he has restored my quality of life through challenging spine surgeries. He has always started with conservative approaches, striving to avoid surgery yet when surgery was clearly required, he performed very skilled, compassionate surgeries on my cervical, cervical fusion and lumbar which allowed me to continue a highly active, unimpacted quality of life. In all cases, other surgeons recommended more aggressive surgeries and Dr. Melamed always opted for just what was needed (e.g. laminotomy vs. laminectomy) and this always proved to be best for my health and wellbeing. I am a retired firefighter with 29 years of service. I was forced to retire because of back injuries I sustained during my career. Over the years, Dr. Melamed has performed three lumbar procedures on me, and in 2024, he performed a triple cervical disc replacement. Dr. Melamed is an outstanding orthopedic surgeon, but what truly sets him apart is the way he advocates for his patients. Every one of my procedures was initially denied by Workers’ Compensation. Rather than simply accepting those decisions, Dr. Melamed personally contacted the doctors who had denied my treatments and fought to have those decisions changed. Because of his efforts on my behalf, I was able to receive the treatment I truly needed. Dr. Melamed went above and beyond to make sure I received the care I needed. Not many doctors are willing to take that extra time and personally fight for their patients. He did exactly that for me. I wish there were more doctors like Dr. Melamed—doctors who are ethical, compassionate, dedicated, and willing to do the right thing for their patients.
